Last Minute Risk Analysis
Before every task, and again when the situation changes, ask yourself four questions. It takes a minute and can save a life.

What am I going to do?
Do you know the task, the work permit and the work instruction?

What can go wrong?
Look around: height, energy, substances, traffic, suspended loads.

What do I do about it?
Take measures and use the right PPE and equipment.

Can I start safely?
In doubt? Then don't start and consult your supervisor.
Reporting
What do you report?
Every report helps. You will never be blamed for a report, only thanked.
Unsafe situation
For example a broken scaffold railing or an unguarded open pit.
Unsafe act
Someone working without PPE or without a permit.
Near miss
It almost went wrong. That is exactly where we learn the most.
Accident or incident
Always report immediately to your supervisor and the QHSE department.
